Disclaimer. Garage Stories (the Event) and RLab (the Location) staff reserves the right to remove any person (or persons) who presents potential  harm, danger or disruption to themselves, anyone else or the Locations while attending the Event.

 

Project Rights. 

(1) I hereby affirm that I will secure all rights (including without limitation copyrights) in and to all material included in my project materials, including without limitation any and all images, photographs, performances, appearance(s) by any person(s), sounds, soundtrack and music (including without limitation compositions, recordings and performances).

 

(2) I hereby affirm that I will grant to Garage Stories non-exclusive rights over my project to screen, show, exhibit, telecast, broadcast, advertise, and disseminate the Project and/or any portion thereof throughout the universe in perpetuity, via any medium and in any manner whether now known or hereafter invented. Garage Stories shall have the right to use the Project and any portion thereof to advertise and promote itself. Garage Stories do NOT have the right to sell or use the project for commercial purposes without a previous agreement.

 

Photo & Video Release. Subject to the terms of this Form and without expectation or obligation of any compensation from Garage Stories, I hereby give my consent to Garage Stories and its affiliates to use my name, image and likeness and/or any interview statements  from me in its publications, advertising, training or other media activities (including the Internet and social media sites such as Facebook, Twitter, Instagram, etc.). This consent includes, but is not limited to my permission to film,  photograph, tape, or otherwise make a video of me and use such audio/visual materials and/or quotes from the interview(s) in training materials, mailings and any other print and electronic media now known or hereinafter devised. This consent is given in perpetuity, and does not require  prior or any additional approval by me, nor is there any geographic limitation on where these materials may be distributed and applies to all such materials collected in the past, the present or the future as part of any dance audience or auditions, interviews, classes, sessions, workshops or events attended  by me as a result of, or through my association with, Garage Stories and their affiliates.

 

Liability Release. For good and valuable consideration, the receipt and sufficiency  of which are hereby acknowledged, I agree to release and hold harmless Garage Stories (the “Event”), and the RLab, the facilities on whose premises the Event I am attending will occur (the “Location”), and the respective directors,  officers, representatives, members,  agents , affiliates and employees of the Event and the Location and their respective affiliates (hereinafter collectively “ Releases”)  from any and all liability, whether  based on negligence, contract, tort,  statute or otherwise, for any claim, judgment, loss, liability, cost and expenses (including,  without limitations, attorney’s fees and costs) arising out of or connected with the Event I am attending, including but not limited to any claim arising out of or connected  with any illness or injury (minimal, serious, catastrophic and / or death) that I may incur or sustain during the Event, all activities associated with the Event and while traveling to and from the site for the Event whether or not the Event actually  occurs. I further expressly agree to indemnify and hold harmless Releases and Releases’ heirs, successors, assigns, executors and administrators against loss from any further claims, demands or actions that may subsequently be brought by me or by any other persons  on the account of damages of any character resulting to me in any way from the foregoing activities. I further agree to reimburse and to make good to Releases any loss, damages, or costs Releases may have to pay as a result of any such action, claim, or demand, including but not limited to attorney's’ fees.

 

Medical Release. I acknowledge  and agree that participation in the Event subjects me to the risk of physical illness or injury (minimal,  serious, catastrophic and / or death) and that I acknowledge that I am assuming the risk of such illness or injury by participating  in the Event. In the event of such illness or injury , I authorize PTC to obtain necessary medical treatment for me and hereby release and hold harmless  Releases in the exercise of this authority. I further acknowledge and understand that I will be responsible for any and all medical and related bills that may be incurred by me for any illness or injury that I may sustain during the Event and while traveling to and from the site for the Event whether or not the Event actually  occurs.

 

I hereby warrant that I have read this Release Waiver Form in its entirety and fully understand its contents. I am aware that this Release Waiver Form releases Releases  from liability and contains an acknowledgement of my voluntary and knowing assumption of the risks of injury or illness. I have signed this document voluntarily and of my own free will.
